Hospice Survey 2021/22 - Standalone Registration


Welcome to the 2021/22 Hospice Survey registration

Over the next few pages you will be able to register your Hospice to take the 2021 Hospice Survey on a standalone basis. The registration form will allow you to customise the survey for your specific analysis requirements.

There are four parts to the registration:
  1. Details of your hospice and your overall requirements for the survey.
  2. Selection of the reports you would like to receive.
  3. If staff and volunteers are taking part in the survey, you will be ask to identify who to include in each of your chosen reports - just staff, just volunteers, both or separate reports for each group.
  4. The time of year* you would like your survey to run, and any other requests. 

Information about the cost of your survey:
The core cost of taking the Hospice Survey is £475+VAT, including the Overall Results report. You can add further options to this core service if you would like - and this registration form will guide you through those. Our prices have not increased since 2019.

Please note that all prices shown in this registration exclude VAT which is charged at the current rate of 20%.

*Note regarding the timing of your survey:
At all times of year you need to register at least three weeks in advance of your chosen survey launch date. At some times of year we are already quite booked up and we may not be able to allocate you the specific survey launch date that you request - although we will always try our utmost to do so.

The document also gives you information about the updates and changes that have been made over the past year.
